Instrument Usage Fee and Contract*

*The $75 Instrument Usage Fee and Contract is only Colorguard, Percussion, and for students using PFISD-owned instruments. It is due May 1, 2019 for all students enrolled in band for 2019-2020. Forms and fees should be submitted prior to the Spring Marching Camp May 1st. This includes:​

Temporary Employee Forms

Parents, please fill out the Absence Request Form for every absence. It requires the date of the absence to be listed and an electronic parent signature. Additionally, please email the applicable director (guard, percussion, band, etc) and inform the director of your request face to face. Communication is always appreciated with at least one week's notice.

Member Forms


All forms must be up-to-date and fully completed. If forms are out of date, students may not travel with the band
