Parents, please fill out the Absence Request Form for every absence. It requires the date of the absence to be listed and an electronic parent signature. Additionally, please email the applicable director (guard, percussion, band, etc) and inform the director of your request face to face. Communication is always appreciated with at least one week's notice.
All forms must be complete and up-to-date. If forms are out of date, students may not be allowed to rehearse, perform, or travel with the band.

Equipment Usage Form (due May 1, 2019)
The Equipment Usage Form and $75 Usage Fee are for all color guard and percussion students as well as woodwind and brass players using PFISD-owned instruments, including:
The form and payment (checks payable to HHS Band) are due May 1, 2019 and covers equipment usage through the end of the 2019-2020 school year. Students who use multiple instruments (such as a mellophone for marching band and a horn for concert band) need only fill out the form and submit payment once.
PfISD Physical Examination Form (due July 1, 2019)
Per PfISD policy, all students in the Hawk Band will need to have a physical on file, renewed each year. Students who are involved in athletics may use the same physical for the band. There will be a clinic hosted at a PfISD school who students and families can do walk-in physicals at a future date to be determined.
Hawk Band Student Forms (due August 1, 2019)
This packet includes the Band Handbook Acknowledgement Form, UIL 8 Hour Acknowledgement Form, Photo Release Form, and the Travel and Medical Release Forms.
